We call Abdominoplasty the procedures needed by the patients, men and women, for their abdominal region. The causes for abnormalities or deformities in this region are numerous. In women these are:
1. Pregnancy
2. Poor body posture
3. Sedentary Lifestyle
4. Obesity, morbid or not
5. Genetic constitution
6. Loss of weight after a bariatric surgery
For men the causes are very similar except for pregnancy. The majority of these patients have localized adiposity which needs liposuction. Therefore these two procedures (Abdominoplasty and Liposuction) generally are done together to obtain a more refined result.
As in any surgical procedure its important to obtain a thorough Medical History and a Physical Exam to determine the possibility of Deep Vein Thrombosis, prolonged use of contraceptives, presence of varicose veins in the lower limbs, presence of abdominal hernias and previous scarring due to surgery. It’s important to evaluate the how flaccid the skin is and also the presence of diastasis or weakening of muscular walls. Depending on the results of the Physical Exam we can treat the abdomen and the surrounding area in these ways:
1. If the patient is overweight with localized adiposities, without flaccid skin or muscle, the procedure would be Liposuction and a healthy diet.
2. If the patient is overweight with localized adiposities, with flaccid skin or muscle, then the procedure would be Liposuction and Abdominoplasty. The resection of the skin can be complete or incomplete and depends on the excess. One must keep in mind that good scarring can be expected and depends on the magnitude of the tension on the scarring line. The extra weight can be corrected with a healthy diet.
3. If the patient is not overweight, but does have flaccid skin and muscle then the procedure would be an Abdominoplasty.
4. If the patient is not overweight and does not have flaccid muscle but does have flaccid skin, then the procedure would be resection, complete or not.
5. If the patient is not overweight and does not have flaccid skin but does have flaccid muscle then the procedure would be to reinforce the abdominal wall with a small resection of skin.
As you can see there are multiple procedures and thus the Physical Exam is very important. The belly button is very important (Umbilicoplaty) because it is an essential part of the overall aesthetic look of the abdomen and the Plastic Surgeon should be careful not to make it too big or too small.
During the procedure, especially in overweight patients, the use of compression socks is highly recommended. During the post-opt the use of anticoagulation medication of low molecular weight as a prophylactic (such as Clexane) and an early deambulation minimizes most of the problems associated with thrombosis that the patient might have. Antibiotics, Pain Relievers and Anti Inflammatory medication, waist band and drainage tubes are mandatory for an optimal recovery.
